Eligibility, briefly
A prognosis of six months or less if the illness runs its usual course, certified by the attending and our medical director, and the patient elects the hospice benefit. If you are unsure whether a patient qualifies, call and we will review it with you.

Questions we get from referral sources
What do you need from me to take a referral?
Patient name, date of birth, the referring physician, the terminal diagnosis and a contact for the family. Send it by phone or secure fax. We handle certification and the face-to-face requirement from there.
How fast do you respond?
Same-day admissions. Refer today and we admit within 24 hours, including weekends. You will get a call back rather than a queue.
Can the attending physician stay on the case?
Yes. The attending can remain the patient's designated physician and continue billing under Part B while our medical director oversees the hospice plan of care.
Do you take patients already in a facility?
Yes. We serve patients in private homes, assisted living, memory care and skilled nursing facilities across our service area.
